The real constraints
Not safety. Not food. Three things:
- Road days. Distances are short on the map and long in hours. A six hour drive on a winding mountain road is harder on an older body than an eight hour drive on a motorway, and most Northeast itineraries are built by people who did not notice.
- Steps. A great many sights end in a long flight of uneven steps with no handrail.
- Medical distance. Outside Guwahati and Shillong, a hospital can be hours away.
Altitude is a fourth if Sikkim or Tawang is on the list.
What works
Kaziranga. The safari is done from a vehicle. Lodges are on flat ground. The experience is genuinely world class and demands almost nothing physically. Kaziranga safari guide.
Shillong and Umiam Lake. A comfortable base, decent hospitals, short drives to viewpoints, and Elephant Falls is close to the parking.
Gangtok town. Flat-ish along MG Marg, good hotels with lifts, and the cable car covers ground that walking would not.
Majuli. Slow, flat, and the sattras are approachable. The ferry crossing is the only real obstacle. Majuli island travel guide.
Guwahati. Underrated as a base. Kamakhya has steps, but the city itself is easy and everything else is within reach.
What to skip
- Nongriat double decker root bridge. Around 3,000 steps down and the same back up. It is harder coming up than going down, and the step counts quoted online vary wildly. This is not a negotiable one.
- Nathu La and Tsomgo Lake for anyone with a heart or breathing condition. Over 4,000 m, reached quickly by road.
- Sela and Tawang unless you have several days to ascend slowly.
- Dzukou Valley. A proper trek, not a walk.
How to build the itinerary
Three hours of driving a day, maximum. Four if it is unavoidable, never two days in a row.
Two nights minimum everywhere. One-night stops mean packing every morning, which is where fatigue actually comes from.
A rest day in the middle. Not a sightseeing day with a lie-in. A day where nothing is scheduled.
Book ground floor or a lift. Ask specifically. Many Northeast hotels are three or four floors with no lift and will not mention it.
Private vehicle, not shared. The cost difference is real and worth it. A shared sumo does not stop when someone needs it to.
A workable seven days

Day 5 is the one to watch. If it is too much, add a night at Guwahati and split it.
Practical
- Carry all medication in original packaging with prescriptions, plus a written list of what they take and the doses.
- Note the nearest proper hospital for each stop before leaving.
- Travel insurance that covers evacuation, not just treatment.
- October to March for weather, and avoid the monsoon entirely for this kind of trip.
